Transitional Care Unit

AT the Transitional Care Unit of the Livingston County Center for Nursing & Rehabilitation, our Team of Therapeutic Specialists strive to create a restorative environment where individuals can reach their maximum potential. We believe that successful restorative therapy should be as diverse as the patients and community we serve.

GOALS
With self-reliance and independence as our restorative foundation, we customize your restorative care plan to fit your individual needs.

TEAMWORK
At the Transitional Care Unit, a multi disciplinary team of experienced professionals work together to provide the best possible restorative environment. We believe that the family and patient are equal partners in this team. During a patients restorative stay, we will continually educate and inform to ensure long term restorative success.

EDUCATION
A personalized restorative treatment program is initiated immediately upon admission. As a patient progresses, this plan grows and evolves to constantly promote and maximize the restorative environment. Regular conferences with the care team are held to educate and review the patients progress.

PROGRESSION
Our Community Re-entry Program will ensure that each patient has a safe transition back to their residence. Members of our therapy team may visit a patient's home during treatment and recommend modifications to address special needs.
